Death Certificate Amanda KERNS CHAMBERS

Missouri State Board of Health, Standard Certificate of Death. Amanda Chambers died at 7:30pm on 8 January 1941 at age 88 years 5 months 23 days of chronic myocarditis. At the time of death, she was a widow of  Lewis Chambers, and was a retired homemaker. Amanda was born 15 July 1852 in Hemple, Missouri to James Kerns (born in Hemple, Missouri) and Elizabeth Boyer (born in unknown town, Pennsylvania). Informant was Mrs. Elizabeth Sherwood, R.F.D. #1, Clarksdale, Missouri. Burial was in the Freeman Chapel Cemetery 10 Jan 1941.  Amanda's usual residency is R.F.D #2, Easton, Buchanan, Missouri. Death was in Rural Sherman, R.F.D. #1 Clarksdale, DeKalb, Missouri, and had been living in this location for 2 years. No Social Security Number listed. Doctor was O.L. Perkinsy of Clarksdale, Missouri. Certificate number 2419 was filed 13 January 1941 by Mrs. C.M. Davis, registrar.
